Select the appropriate questionnaire and provide biographical information on yourself or your child. You might think that some of the information requested is irrelevant. However, we do ask that you print out the questionnaire and fill in as much information as you can and as much information as you feel comfortable providing, then bring it with you to your first session. We believe that it will help the doctor arrive at the most accurate diagnosis possible and therefore, provide you with the most efficient treatment. Some of the information isn’t really related to treatment at all, as you might guess, but rather, is just good information for the clinic to have on hand in case of any emergencies.
